Short answer
Amazon does not send a notification when the FBA fee on one of your ASINs changes. Notifcentral pulls your fee estimates from the Selling Partner API every six hours, compares each ASIN against its previous snapshot, and pushes an alert showing the old amount, the new amount, and the per-unit difference. What it costs to find out late
A wrong measurement can double a fee.
Sellers on the Amazon Seller Forums keep reporting the same problem. A warehouse measures a product into a higher size tier, Amazon sends no alert or email, and the seller finds out months later. One seller on Reddit said two SKUs were charged at the wrong tier for five months before anyone noticed.

Amazon will re-measure on request and can reimburse an overcharge. The case is far easier to argue while the change is recent and you still have the numbers.
What to do when it fires
Check the dimensions Amazon has on file
Open the ASIN in Seller Central and compare Amazon's recorded length, width, height and weight with the packed product. If the size tier went up but your packaging did not change, the measurement is probably wrong.
Request a remeasurement (Cubiscan)
If the recorded dimensions are wrong, open a case asking for the unit to be re-measured. Amazon runs the product through a Cubiscan and updates the record.
Ask for the overcharge back
Once Amazon confirms the corrected dimensions, ask for reimbursement on units already fulfilled at the wrong tier. Include the date the fee changed, which is in the alert.
Re-check the margin at the new fee
If the new fee is correct, check whether the ASIN still makes your minimum margin at that fee, then decide whether to reprice or delist.
